The Team Care Clinic is an interprofessional education and collaborative practice model that provides family medicine residents in the IU Health/Methodist Family Medicine Center the opportunity to see scheduled patients within a team-based approach. Other healthcare disciplines include chaplaincy, nursing, pharmacy, physical therapy, and social work. The team-based care continuum ranges from interprofessional coordination to interprofessional collaboration utilizing teamwork as the premise for patient-centered care. Interprofessional communication is highly valued for brief hand-offs as team members see each patient individually following a team huddle to ensure shared responsibilities and optimal clinic flow.
